Medication Consent Form (PDF)
In order for DEDP to administer ANY type of medication to your child, we are required by law to have this form on file for your child.

Individual Health Care Plan Form (Epi Pens and Inhalers, PDF)
If your child has a medical condition that requires an epi pen or inhaler, please complete this form in addition to the medication consent form.

EEC Emergency Care Plan (PDF)
This form will give us more information on your child's allergies.
